Migraine Diaries

Migraine diary cards which record information about the attacks of migraine and other headaches are valuable in assisting the doctor with diagnosis, assessing trigger factors and assessing the effectiveness of treatments.

At the City of London Migraine Clinic you will be given diary cards on your first visit and they will be assessed at your follow up visit. If you are able to come to the clinic with diary information at your first visit this will help the doctor to help you.

We use two types of diaries at the City of London Migraine Clinic. Both diaries are very easy to use and you can download them in PDF format along with full instructions by clicking below. If you do not have the adobe acrobat reader software to read the files, this can be obtained from www.adobe.com at no charge.

If you would like us to send you a copy of either or both diaries please write to us enclosing a self-addressed envelope with two first class stamps. As this costs us time and money, a donation is always appreciated.

Annual Migraine Diary

This single sheet diary may be used as a summary of your migraines and headaches over a year. It is particularly useful to record attacks here when you can copy the frequency from another source eg your calendar or personal diary.

For women it can also provide an overview of the relationship between menstrual cycles and migraine.
